BMI for men — three caveats worth knowing

BMI was a 19th-century population statistic, not an individual health score. For men specifically, it has three blind spots.

1. Muscle weighs more than fat

Lean lifters routinely show up "overweight" or even "obese" on BMI alone. A 90 kg, 1.78 m male at 12% body fat is BMI 28.4 — but he has half the metabolic risk of a 90 kg, 1.78 m male at 28% body fat with the exact same BMI. If you train and your BMI looks high, run a body-fat calculation before worrying.

2. After 50, BMI hides muscle loss

Sarcopenia (age-related muscle loss) starts in the late 30s and accelerates after 50. BMI doesn't track this — you can lose 5 kg of muscle and gain 5 kg of fat over a decade and your BMI won't move. Track your body fat % and waist circumference alongside BMI from your 40s onwards; the trend in those two will tell you what BMI is hiding.

3. Indian / South-Asian men — overweight starts at 23

ICMR and WHO South-East Asia guidelines flag BMI ≥ 23 as "overweight" in Indian men (vs WHO's global 25) because of the higher visceral fat phenotype. If your BMI is 23–24.9, check waist: over 90 cm in Indian men is the threshold where insulin resistance and fatty liver typically start to show in bloodwork.
